ACA in South Dakota: How the South Dakota Marketplace Works

South Dakota buyers use the federal marketplace, HealthCare.gov, because South Dakota runs no state marketplace of its own. South Dakota expanded Medicaid by ballot initiative, Amendment D, in November 2022, with expansion taking effect 1 July 2023 — the most recent expansion among the states on this page. You create an account, enter household and income details, and the marketplace shows your eligibility result — marketplace cover with or without a tax credit, or a referral to Medicaid or CHIP where that fits.

South Dakotans buy marketplace coverage through HealthCare.gov, the federal marketplace, which routes you by ZIP code to the plans available where you live.

How the South Dakota marketplace works

Key points

  • South Dakotans buy marketplace coverage through HealthCare.gov, the federal marketplace because South Dakota runs no state marketplace of its own. Preview plans for your own ZIP code.
  • The 2026 benchmark premium in South Dakota is $655 a month (KFF, 2026) — the figure the premium tax credit is calculated from. It is a statewide average for the benchmark plan as defined in the data file; your area's figure comes from the marketplace preview for your ZIP code.
  • South Dakota has expanded Medicaid, so adults can qualify on income up to 138% of the poverty level.
  • Check four things before you enroll: that your doctors and hospitals are in network, that your regular medicines are covered and on what tier, that the deductible and out-of-pocket maximum fit a year in which you actually need care, and that you understand what the plan excludes.

How the South Dakota marketplace works

You create an account, enter household and income details, and the marketplace shows your eligibility result — marketplace cover with or without a tax credit, or a referral to Medicaid or CHIP where that fits. The application, not a third-party estimate, is the decision.

Plan availability varies by area, so a plan a friend has in another South Dakota city may not be offered where you live. Preview plans for your own ZIP code and check each plan's network for your doctors and hospitals before you compare premiums, because a plan that excludes your regular care is not a bargain at any price.

South Dakota expanded Medicaid by ballot initiative: voters approved Amendment D in November 2022, and expansion took effect 1 July 2023, covering adults aged 19 to 64 up to 138% of the poverty level. That date matters because older guides still describe South Dakota as a non-expansion state; for 2026 coverage it is an expansion state.

Benchmark premium in South Dakota

The 2026 benchmark premium in South Dakota is $655 a month (KFF, 2026) — the figure the premium tax credit is calculated from. The figure is the second-lowest-cost silver (benchmark) premium for a 40-year-old in each county, weighted by county plan selections, as published by KFF for 2026, and it may include premiums for non-Essential Health Benefits. It is a statewide average, not a single county's premium and not a single plan's price.

That distinction is the whole point of the figure: it sets the tax credit, it does not set what you will pay. Your own premium depends on your age, where you live, the plan you choose and the tax credit you qualify for, so run the ACA subsidy calculator for a planning estimate and let the marketplace application give the official figure. The all-states benchmark table puts the South Dakota figure beside the other states.

Subsidies in South Dakota

Subsidy help in South Dakota follows the same national logic as everywhere else: household size, expected income and the benchmark premium for your area shape the premium tax credit, and cost-sharing reductions attach to silver plans for eligible buyers. The benchmark premium on this page is the figure that calculation starts from.

Run the ACA subsidy calculator first to get an educational estimate, then let the marketplace application give you the official figure. Complete the application even if you assume your income is too high for help: the application is the decision, and a household that guesses wrong can miss a credit it qualified for.

Income that changes during the year should be reported when it changes: updating promptly is the simplest way to avoid a repayment surprise when the year's actual income differs from the estimate you enrolled on.

Medicaid expansion in South Dakota

South Dakota has expanded Medicaid, so adults can qualify on income up to 138% of the poverty level. That expansion is the route for many lower-income South Dakotans: if your income sits below the subsidy band, the marketplace application can refer you to South Dakota Medicaid instead of leaving you to buy a marketplace plan with no help. Apply anyway and let the application sort it — the application itself is the reliable test, not a guess about where your income falls.

Medicaid and marketplace cover are different routes to the same end, and the boundary between them is your income, not your preference. If your income rises during the year past the Medicaid line, report the change promptly so the marketplace can move you to a marketplace plan with any tax credit you qualify for, rather than leaving a gap. The Medicaid eligibility guide explains the programme in more detail.

Open enrollment and renewing for the next year

For 2027 coverage, open enrollment runs November 1, 2026 to January 15, 2027. Enrol by December 15, 2026 for cover starting January 1, 2027; enrolling by January 15, 2027 starts cover on February 1, 2027. Outside open enrollment, a qualifying life event — such as marriage, birth or adoption, losing other cover, or moving — opens a 60-day special enrollment window.

Marketplace cover does not carry over unchanged. Each year, update your expected income and household details, then compare plans and renew your current plan if it is still available, or choose a different one. If you do nothing by the December deadline, you may be automatically re-enrolled for cover starting January 1 — but the help is based on your expected income for the year ahead, not last year's, with the difference settled at tax time. Two letters, one from your insurer and one from the marketplace, arrive by November 1 and show whether your plan is available next year, so read them before you decide.

What to check before enrolling

Check four things before you enroll: that your doctors and hospitals are in network, that your regular medicines are covered and on what tier, that the deductible and out-of-pocket maximum fit a year in which you actually need care, and that you understand what the plan excludes. The health insurance exclusions guide lists the common exclusion types to look for in the plan documents.

Enrolling without paying the first premium leaves you uninsured, so pay it and keep the receipt. If two plans look close on premium, compare them on total cost with the health cost estimator. Marketplace local-help finders point to free navigators and certified helpers in your area — you do not need to pay anyone to complete a marketplace application.

If your income suggests Medicaid may fit, read the Medicaid page too — South Dakota rules for adults are state-specific, and the application itself is the reliable test. For a total-cost comparison of two plans, use the health cost estimator alongside the ACA subsidy calculator.

Keep your marketplace confirmation and first-premium receipt together. If you need help, the marketplace's local-help finder points to free navigators and certified helpers in your area.

Worked example

Ethan, a fictional South Dakotan, previews plans for his ZIP code, enters household and income details, and shortlists two plans. He checks both plans' networks for his doctor and his regular medicine tier before choosing, and pays the first premium to activate cover. No South Dakota rule, date or plan price in this example is asserted as a fact.
