How the Illinois marketplace works
You apply on Get Covered Illinois, enter household and income details, and the exchange shows your eligibility result — marketplace cover with or without a tax credit, or a referral to Medicaid or CHIP where that fits. The application, not a third-party estimate, is the decision. Plan year 2026 was the first year Illinoisans enrolled through Get Covered Illinois, after the state moved off HealthCare.gov.
Plan availability varies by area, so a plan offered in one part of Illinois may not be offered where you live. Preview plans for your own ZIP code and check each plan's network for your doctors and hospitals before you compare premiums, because a plan that excludes your regular care is not a bargain at any price.
Because 2026 was the first Get Covered Illinois plan year, the state ran a limited-time transition special enrollment period for people who had been auto-renewed from a HealthCare.gov 2025 plan and had not claimed their Get Covered Illinois account by February 1, 2026.
Benchmark premium in Illinois
The 2026 benchmark premium in Illinois is $646 a month (KFF, 2026). Premiums were analyzed using the second-lowest cost silver (benchmark) premium for a 40-year-old in each county and weighted by county plan selections, and may include premiums for non-Essential Health Benefits. It is the figure the premium tax credit is calculated from.
That figure is a statewide average, not a quote and not what any individual will pay. Your own premium depends on your age, household, income, county and the plan you choose, and the exchange application gives the official figure for your household. Use the subsidy calculator for a planning estimate, and the all-states table at average insurance costs by state to see how Illinois compares with other states.
Subsidies in Illinois
Subsidy help in Illinois follows the same national logic as everywhere else: household size, expected income and the benchmark premium for your area shape the premium tax credit, and cost-sharing reductions attach to silver plans for eligible buyers.
Run the ACA subsidy calculator first to get an educational estimate, then let the exchange application give you the official figure.
Income that changes during the year should be reported when it changes. The reconciliation at tax time is the same national rule, and updating promptly is the simplest way to avoid a repayment surprise.
Medicaid expansion in Illinois
Illinois has expanded Medicaid, so adults can qualify on income up to 138% of the poverty level. If your income is near that line, apply anyway and let the application sort it — the application itself is the reliable test.
Illinois' expansion took effect on January 1, 2014.
Open enrollment and renewing for the next year
Open enrollment for 2027 coverage on Get Covered Illinois runs November 1, 2026 to January 15, 2027. Enrol by December 15, 2026 for cover starting January 1, 2027; enrol by January 15, 2027 for cover starting February 1, 2027. Outside open enrollment, a qualifying life event — marriage, birth or adoption, losing other cover, or moving — opens a 60-day special enrollment window.
Marketplace cover does not simply carry over unchanged from one year to the next. Each year, update your expected income and household details on your application for the coming year, then compare plans and renew your current plan if it is still available, or choose a different one that fits better. Check Get Covered Illinois' renewal notices before you decide whether to renew or switch, and confirm the current window on the exchange — dates can change.
What to check before enrolling
Check four things before you enroll: that your doctors and hospitals are in network, that your regular medicines are covered and on what tier, that the deductible and out-of-pocket maximum fit a year in which you actually need care, and that you understand what the plan excludes. Enrolling without paying the first premium leaves you uninsured, so pay it and keep the receipt.
Illinoisans who were auto-renewed from a HealthCare.gov 2025 plan should check that they have claimed their Get Covered Illinois account — the state's transition safety net for unclaimed accounts was time-limited.
Illinois links and next steps
If your income suggests Medicaid may fit, read the Medicaid page too — Illinois rules for adults are state-specific, and the application itself is the reliable test. For a planning estimate, run the ACA subsidy calculator; for a total-cost comparison of two plans, use the health cost estimator.
Keep your exchange confirmation and first-premium receipt together. If you need help, the exchange's local-help finder points to navigators and certified helpers in your area, and that help is free — you do not need to pay anyone to complete a exchange application.
Worked example
Marcus, a fictional Illinoisans, previews plans for her ZIP code on Get Covered Illinois, enters household and income details into the subsidy calculator, and shortlists two plans. She checks both plans' networks for her doctor and her regular medicine tier before choosing, and pays the first premium to activate cover. No Illinois rule, date or plan price in this example is asserted as a fact.
Illustrative example only — not a fact about any real policy.
